Etymotic FTW! I have had mine for 5 years and they are still the best in-ear headphones I have ever had. They opened up a whole new world of sound to me. You would never regret getting a pair.

Not with any unit of any quality. The only time I've ever run into quality issues with mine is when I'm talking to someone with the phone a significant distance away from me (like 20+ feet).
