It certainly did leave the factory like that. 
Of course not back in 1995 when it first was built though.
This F1 is chassis #040 - the first McLaren F1 built with the high mirrors at the request of its original owner. It was originally painted silver, then when the current owner purchased it about a decade ago he had it resprayed in that medium blue linked above. Then after his small shunt on the Autobahn last year he chose to have the new color applied and several other tweaks for personalization, including the doodle. All the work was done in house by McLaren except for the graphic - the owner brought in someone who specializes in pinstriping to handle that.
He had toyed with some other ideas for the small graphic too before deciding on the final version:

The tattoo/squiggle actually looks really really good in place (vs. those bonnet only shots we saw a while back). It is not in-yer-face and doesn't look like a scratch in the paint - it's almost invisible when there are a lot of reflections, and very clear when there are not - hopefully some of the pics show that. I was really skeptical, but think it is a very nice touch, very individual.
