I know fuel econ, Vanos and SMG Transmissions are a big concern with these cars for most people, what else is there to look out for?

That motor's sound is to die for

How sad for the glorious E60. How many miles for one of those?
Think you hit the big 3 red flags. Can't think of any recurrent major issues other than those.

I think a bit higher for low mileage, full spec car. Warranty is extra. Don't go cheap, it will break, and you'll go big on expensive repairs.
Get a 2008+ car if you want more peace of mind.

With a V10 that gets 10 to 12 mpg and gas at over $4 a gallon and rising it was only a matter of time.

Love the E60 M5 and I wanted that car soo bad when I was in college. Now I can get one anytime I want I just can't afford the gas I think the gas bills will be higher than the car note

No worse than driving an SUV these days.
On top of that you got 500 hp on top.
