Folders by genre on my mac pro desktop at home, backed up to two separate 500gb external drives via OSX time machine for redundancy. Use itunes to make a playlist for each genre and from there create playlists for what ever i feel like listening to on the ifone and ipod nano.
Yea, probly a lot more work than it needs to be but my tunes are important to me so i dont mind. Been doing this since '97 since there werent many decent tools at the time for Mac's and many of my mp3s dont have complete id3 tags either which makes them invisible to itunes automatic sorting features.

If you rip your own CDs to mp3 make sure to put in the id3-tag. Or if you download them, just download a proper release and the id3-tag will be there.
I just keep them all in a folder sorted by artists - album. Usually have all the music loaded in Winamp anyways.

One big 32Gb mess.
I used iTunes at some point, and had my songs sorted with the stars thing. 5 stars was an exceptional song and one star was "don't even try to listen". No star was "not rated yet". Then I changed HD ... and lost all the ratings.
So now it's just one big mess.
