Thailand Guide Recommendation Needed

We are doing a trip to Thailand in a few months. There will be two couples for a total of 4 people. We are perfectly happy arranging our own car, or hiring a guide who comes with a car. Does anyone have recommendations for guides in the following areas:

Bangkok (+/- 3 or 4 days)
Plan is to hit, among other things:
Ayutthaya History Park
Amphawa floating Market
Grand Palace/Wat Phr Kaew/Emerald Budda

Chiang Mai (+/- 3 days)
Chiang Dao Elephant Training Camp
Wat Phrathat
Meo village
Cooking class (No idea where, but a friend recommended we find one)

Udonthani (+/- 3 days)
Don't need help here, we'll be visiting a friend's family.

Koh Lanta (+/- 5 days)
We'll have a car here and are planning on being lazy around the beach, but if there are must-see's we'd like to have a guide.

Thanks in advance for any suggestions for a good guide for each locale you may have.

Wound up with excellent guides in Bangkok (referred through a friend) and Chiang Mai (referred through a cooking school I booked directly) and a boat to hire for the day in Koh Lanta.

Can't recommend the Rachamanka Hotel highly enough as a great place to stay in Chaing Mai.
