Well that didn't take long. Yesterday was the first day Tesla Model S owners could download the Autopilot software upgrade to make their cars semiautonomous, and already we have one example of an owner almost running off the road because they didn't understand the system's limitations.

When one Tesla Model S owner returned to his parked car, he found it smashed up and severely damaged.

The owner says that he was answering questions about the car from a passerby, so it's entirely possible that he opened the app and accidentally activated Summon (for what it's worth, he claims he didn't). But here's the thing: it doesn't matter. Under no circumstances should a production vehicle autonomously collide with a parked vehicle mere feet ahead of it. I think that's common sense, right? Am I saying anything controversial?

Tesla’s autopilot failed to brake; Tesla says otherwise - May 13, 2016 10:30pm CEST

On April 26, Simpson was driving north from Los Angeles on I-5, cruising in autopilot mode. "All of a sudden the car ahead of me came to a halt. There was a decent amount of space so I figured that the car was going to brake as it is supposed to and didn't brake immediately. When it became apparent that the car was not slowing down at all, I slammed on the brakes but was probably still going 40 when I collided with the other car," she told Ars.

In contrast, Tesla says that the vehicle logs show that its adaptive cruise control system is not to blame. Data points to Simpson hitting the brake pedal and deactivating autopilot and traffic aware cruise control, returning the car to manual control instantly. (This has been industry-wide practice for cruise control systems for many years.) Simpson's use of the brake also apparently disengaged the automatic emergency braking system, something that's been standard across Tesla's range since it rolled out firmware version 6.2 last year.

In his mirror he saw a headlight and dented the hood of the Tesla, which was hooked under the bumper of the truck. The truck hit badly damaged in the collision.

He had, he told the truck driver, driving on autopilot.

The driver concedes:

“First of all, it is clear to me that I am responsible and the accident could have been prevented. But still I see the contributory negligence of the systems. If I can not trust to a certain degree a system, then I can not use the system and it has no value for me.

The Model S was on Autopilot, or at least the Traffic-Aware Cruise Control (TACC) feature of the Autopilot, in the left lane on the highway (Switzerland) in heavy traffic.

Tesla has made a statement saying that they checked vehicle data logs and that the Model X was operating under manual control (not operating in Autopilot of cruise) when the accident occurred.

“This could have easily been a fatal accident if the car’s wheels were not turned slightly to the left. If they were straight, it would have gone over the planters and crashed into the store in front of the parking stall and injured or killed the patrons. I am waiting to hear from Tesla whether the accelerator pedal can be depressed by the car electronically similar to gas-powered cars’ pedal being depressed on their own while in cruise control.”
"My wife is 45 years old and has had a driver's license almost 30 years and has a clean record. UPDATE: Tesla has made a statement saying that they checked vehicle data logs and that the Model X was operating under manual control (not operating in Autopilot of cruise) when the accident occurred.
Tesla say the vehicle had been travelling at 6 miles per hour, until the accelerator pedal was increased to full 100%. Tesla says the Model X responded to the driver’s direction as one would expect.

Tesla has access to all customers car data logs? What does it mean to customers legal rights?

Chinese Tesla Driver Blames Autopilot For Crash He Could Have Easily Avoided

A Tesla Model S was involved in a crash in Beijing earlier this week, with yet another driver blaming Autopilot. This is the first alleged Autopilot crash in China, but when you watch the video, it’s amazing it happened at all.

The driver, Luo Zhen, was reported to have his hands off the wheel and had Autopilot engaged, reports ChinaDaily. It reports Luo to have seen the Santana with the naked eye and that there was a reaction time of about five seconds, but the system didn’t see the disabled car and crashed into it.

The accident cost him 50,000 yuan ($7,525) in repairs. However, “believing there are technical bugs in the system of Autopilot, Luo said Tesla should take half of the responsibility, while the other half should be paid by the Santana’s driver for illegal parking.” He criticized Tesla for exaggerating the abilities of the Autopilot functions while only giving scant warning that it’s an assistance system and shouldn’t be fully relied upon.

Trusting that Autopilot could handle the route as it had done before, he reached into the glove box to get a cloth and was cleaning the dashboard seconds before the collision, he said. The car failed to navigate a bend on Highway 175 in rural Kaufman, Texas, and struck a cable guardrail multiple times, according to the police report of the Aug. 7 crash.

Dashcam shows Tesla crash which killed Chinese driver when futuristic electric car smashed into parked lorry.

Report reveals Chinese (23) died in Tesla crash in China in January. Gao Yaning was killed after his vehicle crashed into a road sweeper.
Tesla say they cannot be sure if on "Autopilot" or not and are investigating.

Police investigators concluded that Mr. Gao was responsible for the accident, CCTV reported. But in July his family sued the dealer who had sold the Tesla.
The driver’s father, Gao Jubin, told CCTV he thought his son had been relying on Autopilot to drive the car and so was not watching the road when the crash took place.

Mobileye’s chairman Amnon Shashua said on Wednesday:

Mobileye broke ties with Tesla Motors because the Silicon Valley firm was “pushing the envelope in terms of safety” with the design of its Autopilot driver-assistance system.
In the Reuters interview, Shashua said Autopilot’s capabilities were being touted too highly, too soon.
“No matter how you spin it, (Autopilot) is not designed for that,” Shashua said. “It is a driver assistance system and not a driverless system.”

Only so far, neither the authorities nor Tesla or the witnesses have confirmed whether the vehicle was indeed using the Autopilot function - or, indeed, whether this particular car even has it installed. What has been rumored, however, is that the driver, a 50-year-old man from Vestfold, was busy with his cell phone just before the crash occurred.

According to the source, the trailers holding the signs were dragged for as much as 80 meters (262 feet) before the car came to a stop, suggesting the Tesla had a pretty high velocity at the time of impact. The driver had his license confiscated on the spot for reckless driving.

Tesla reviewed the logs and told us that the accident was the result of the driver pressing on the accelerator pedal.

Then another similar accident happened just last week in Lexington, Massachusetts, where a Model X crashed into a garage door causing damage to the vehicle, the garage and even another car parked in the garage.

Tesla told by German gov't not to use 'Autopilot' in advertising

FRANKFURT -- German Transport Minister Alexander Dobrindt has asked Tesla to stop advertising its vehicles as having an Autopilot function because this might suggest drivers' attention is not needed.

Germany's Federal Motor Transport Authority (KBA) had written to Tesla to make the request, a transport ministry spokeswoman said on Sunday, confirming a report in the Bild am Sonntag newspaper.

"It can be confirmed that a letter to Tesla exists with the request to no longer use the misleading term Autopilot for the driver assistance system of the car," she said in a written response to a Reuters' query.

Police on the scene did not arrest or charge the Model S driver at the time of the incident. Neither drugs nor alcohol were believed to play a role in this accident.

It seems likely that the driver mistakenly hit the accelerator pedal when trying to apply the brakes. This is now the second time a Model S has crashed into a restaurant. In the previous instance, the Model S driver blamed it on unintended acceleration.

Two people were killed in a fiery electric-car crash that left Tesla in pieces strewn over 150 yards near Downtown early Thursday.
The woman driving the Tesla died at the scene. A man in the passenger seat was taken to Eskenazi Hospital, where he died.

A witness said the car was speeding when it lost control at the intersection, crashed into a tree, and caught on fire. A red Model S hit a roadside lamp pole and then a parked bus.

Witnesses at the scene, quoted by local media, say the driver of the Tesla lost control of the vehicle while he was making a u-turn. The Tesla hit the lamp post at high speed, first up front and then with the right side, and then hit the bus at a relative low speed.

Tesla crashed on a test drive while AutoPilot engaged. Nobody got hurt. But the minor incident gives us a plenty to think about.
The user’s post on the web site’s forum read:
"I was on the last day of my 7-day deposit period. I was really excited about the car. So I took my friend to a local Tesla store and we went for a drive.
AP [AutoPilot] was engaged. As we went up a hill, the car was NOT slowing down approaching a red light at 50 mph. The salesperson suggested that my friend not brake, letting the system do the work. It didn't. The car in front of us had come to a complete stop. The salesperson then said, "brake!" Full braking didn't stop the car in time and we rear-ended the car in front of us HARD. All airbags deployed. The car was totaled. I have heard from a number of AP owners that there are limitations to the system (of course) but, wow! The purpose of this post isn't to assign blame, but I mention this for the obvious reason that AP isn't autonomous and it makes sense to have new drivers use this system in very restricted circumstances before activating it in a busy urban area."

A driver of a Tesla fails to notice at night on the A10 a car arrow trailer on the head. Aftercare of accident caused a lot of congestion on the ring East.

The accident occurred at the height of the exit IJburg/Amsterdam. According to witnesses, the driver braked slightly off the impact, police wrote in a Facebook message.

Both the car and the arrow trailer is total loss. The clean-up and closure of the Zeeburgertunnel caused a huge traffic jam between RAI exit and IJburg. How it is with the driver, is unclear.

A driver of a Tesla fails to notice at 8 o'clock in the evening an engineering vehicle in the North Lantau Highway Hong Kong to the direction of the Kowloon Express line for road works.

A Tesla electric car sailed to the driver suspected not to pay attention to. The front of serious damage, Huangru scrap metal. Two men on Tesla were injured and climbed out of the car and escorted by ambulance to North Lantau Hospital.
