It's both. Some macro photography, likely with a very small camera head or fiber scope setup, and some CG, namely the inside the cylinder bit with the charge going off.
A real air/fuel charge going off results in a very small blue flame front, and a whole lot of non-transparent gasses, not the hollywood-type fireball shown.
Here's an image of a real combustion event taken at high speed, I believe from a Toyota engine design manual I had at some point.
